In-person lifestyle screenings
A lifestyle screening usually includes a questionnaire, cholesterol test, blood
pressure reading, body fat measurement, strength and flexibility tests, cardiovascular
test, and blood test. The results are used by medical professionals to make suggestions
for improvement.
If you are enrolled in Deseret Choice, Deseret Premier, or Deseret Value, you and your spouse
are covered at 100 %, up to $100 per screening. You will be responsible for
your copayment. If you are enrolled in one of our contracted HMOs, please check
with your plan administrator for information about the benefits available to you.
