Deseret Mutual recommends that you follow these immunization guidelines. Please use the following link for a printable form to keep immunization records for your children.

Hepatitis B
First: Birth to 2 months
Second: 1 month to 4 months
Third: 6 month to 18 months
Hepatitis A
First: 2 years
Second: 4 to 6 years
Diphtheria, Tetanus, Pertussis (DPT) - initial series
First: 2 months
Second: 4 months
Third: 6 months
DTaP booster
First: 15 to 18 months
Second: 4 to 6 years
Measles, Mumps, Rubella (MMR)
First: 12 to 15 months
Second: 4 to 6 years
Polio - initial
First: 2 months
Second: 4 months
Polio - booster
First: 6 to 18 months
Second: 4 to 6 years
Haemophilus influenza type B (HIB)
First: 2 months
Second: 4 months
Third: 6 months or 12 to 18 months
Chicken Pox (Varicella) 12 to 18 months or at 11 to 12 years
if no documented immunity
Tetanus, Diphtheria (Td) booster 11 to 16 years, then every 10 years thereafter
