To better manage your health, it's important to assess your current lifestyle. The following tools are a good start as you begin taking better care of your health.

A lifestyle screening usually includes a questionnaire, cholesterol test, blood pressure reading, body fat measurement, strength and flexibility tests, cardiovascular test, and blood test. The results are used by medical professionals to make suggestions for improvement.

If you are enrolled in ManagedCare, NetworkCare or BasicCare, you and your spouse are covered at 100 percent, up to $100 per screenings. You will be responsible for your copayment. If you are enrolled in one of our contracted HMOs, please check with your plan administrator for information about the benefits available to you.
