HOW WE MAY SHARE YOUR PERSONAL HEALTH
INFORMATION
- In early March, 2003 Deseret Mutual will begin implementing new
federal privacy regulations that must be in place by April 14, 2003
regarding your personal health information (PHI).
- You will receive information about our new policies and what you
can do to protect your personal information.
- While these new regulations will not affect your coverage with Deseret
Mutual, they will affect the way we may disclose your personal health
information — either to you or to others on your behalf.
WHAT WILL CHANGE:
- Before we disclose any personal health information, we will ask
questions to verify your identity.
- To request personal health information on another’s behalf
(even immediate family), you must have authorization to access this
information. This may require a password.
- Please keep in mind that you have rights regarding the use and disclosure
of your personal health information. For more information, please
see the Medical Privacy Notice you received in the mail.
