Patient Eligibility

To look up the eligibility status of individuals who indicate they are covered by Deseret Healthcare, enter the patient's Deseret Mutual ID number and date of birth.

Deseret Mutual no longer identifies individuals using the Social Security Number (SSN).

Subscriber Deseret Mutual ID:

Patient Date of Birth: (MM/DD/YYYY e.g. 01/02/1996)
