• Deseret Secure Information (this is a Medicare Private Fee for Service (PFFS) plan)

Deseret Mutual is currently not accepting provider applications for most specialties.

Please Note: Deseret Mutual is currently not contracting with licensed mental health providers, i.e. LCSW, ED.D., Ph.D., LPC, or MFT.  However, you may fax a resume for us to maintain on file 801-578-5916.  If we have need for a particular specialty we will review the submitted resumes, and contact the provider with an application to complete.
