Please Note: Deseret Mutual is currently not accepting provider applications for:

most medical specialties, and
licensed mental health providers i.e. LCSW, ED.D., Ph.D., LPC, or MFT

However, you may fax a resume to us at 801-578-5916 which we will keep on file. If we have need for a particular specialty we will review the submitted resumes, and contact the provider with an application to complete.
