We recommend you review the UHIN HIPAA EDI information for instructions on using UHIN to electronically verify eligibility.

However, if you wish to manually check the eligibility status of individuals who indicate they are covered by Deseret Healthcare, enter the patient's Deseret Mutual ID number and date of birth.

Deseret Mutual no longer identifies individuals using the Social Security Number (SSN).
